CommuniCare+OLE is a network of federally qualified health centers serving underserved populations across Northern California. Established in 2023, the organisation operates 17 clinic sites in Napa, Solano, and Yolo counties, providing care to more than 70,000 individuals. Its model ensures that no one is turned away due to lack of insurance, immigration status, or ability to pay.
The network delivers a broad range of primary healthcare services, including medical care, dental services, optometry, pharmacy, behavioural health and substance-use treatment, and nutrition counselling. Care extends beyond clinic walls through mobile health units, home-visiting programmes, and community and school-based initiatives. Bilingual services are available across its patient population.
CommuniCare+OLE operates under a Patient-Centered Medical Home model, coordinating care across its service lines to support the health of communities in cities including Davis, Woodland, Fairfield, and Napa.
