Functional Industries is seeking a compassionate, creative, and motivated DREAM Enrichment Case Manager to join our team!
In this rewarding role, you will help individuals with disabilities build essential life enrichment skills, foster independence, and experience meaningful opportunities through our DREAM Day Enrichment Program. The DREAM Enrichment Case Manager develops individualized, person-centered plans that focus on personal growth, social connections, daily living skills, and community inclusion. This position also provides leadership and support to staff while coordinating engaging activities, educational experiences, and community outings that enhance the quality of life for the individuals we serve.
Qualifications:
- Bachelor's Degree preferred
- 1–3 years of related experience preferred
- Must meet Minnesota 245D Designated Coordinator qualifications
- Strong communication, organizational, and leadership skills
- Passion for supporting individuals with disabilities and promoting independence
- Valid Minnesota driver's license and ability to travel locally
- Required background and driving record clearances
