SUMMARY DESCRIPTION OF WORK

Perform all aspects of the Intensive Psychiatric Rehabilitation program (IPR) utilizing the rehabilitation technology and philosophy outlined by the Psychiatric Rehabilitation Approach from The Center for Psychiatric Rehabilitation of Boston University to facilitate the achievement of goals identified by individuals authorized to receive these services. The Psychiatric Rehabilitation Practitioner is expected to provide active, skill-based rehabilitation services in both individual and group settings. All necessary training will be provided upon hire.

IPR combines 1:1 rehabilitation services with structured daily and weekly group programming. Practitioners are responsible for creating, planning, preparing, and facilitating engaging groups; teaching and reinforcing skills during community and social outings; helping clients develop and maintain healthy interpersonal boundaries; and meeting required program participation and revenue goals. The role requires creativity, initiative, flexibility, and the ability to turn real-life situations into meaningful opportunities for clients to practice skills related to education, employment, socialization, and housing.

Compensation: between $19.02 and $22.68 per hour (wage is based on total years of relevant experience)
Work Schedule:
37.5 hours per week, Monday through Friday

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree and one year of experience working with adults living with mental illness
  • Documentation of 60 contact hours of Intensive Psychiatric Rehabilitation Service training by a certified instructor, will be provided after hire prior to client engagement.
  • Valid Drivers License and acceptable driving record.

Essential Duties and Responsibilities

  • *Provide all aspects of IPR including: Triage, Readiness Assessment, Readiness Development, Goal Choosing, Goal Achieving and Goal Keeping, to individuals authorized to participate in the IPR program. Services are provided on an individual and group basis while focusing on overall recovery and skill development.
  • *Create, plan, prepare, and facilitate daily and weekly IPR groups that are purposeful, engaging, and connected to clients' rehabilitation goals. Practitioners are expected to demonstrate creativity and initiative in developing activities that promote participation, skill development, problem solving, and recovery.
  • *Plan and facilitate community-based and social outings as active rehabilitation opportunities. While in the community, practitioners must interact with clients, teach and model skills, provide coaching and feedback, and help clients practice communication, social cues, decision making, coping strategies, and appropriate community behavior.
  • *Assist clients with identifying, establishing, communicating, and maintaining healthy personal and social boundaries. Practitioners must be able to address boundary concerns as they arise and use real-life situations as opportunities for teaching, role-playing, problem solving, and skill practice.
  • *Complete all required documentation utilizing MyEvolv including: progress notes, service plans, social histories, certification period reports, IPR forms, discharge summaries, and authorizations, etc., in a manner that meets or exceeds program standards.
  • *Meet established monthly IPR revenue and productivity goals by maintaining an active caseload, providing authorized services, facilitating consistent group programming, and managing time and scheduling effectively.
  • *Attend and participate in individual supervision meetings and all staff meetings.
  • *Communicate with multidisciplinary team members including family members, natural supports and professional supports as appropriate.
  • Provide crisis intervention and planning and problem-solving assistance to clients to assist in accessing services.
  • *Manage all aspects of scheduling for clients including making appointments, communicating changes in schedules, arriving on time for appointments, and supporting consistent participation in authorized IPR services.
  • Assist clients in identifying strengths, needs and goals. Develop an individualized case plan in cooperation with the client. Monitor and evaluate program participant progress and adjust teaching strategies and interventions as needed.
  • *Communicate information to other agencies/organizations as appropriate.
  • *Work both independently, with little supervision, and collaboratively as part of a team. Practitioners must be self-directed in planning groups, organizing community activities, managing caseload responsibilities, and meeting program expectations.
  • *Be dependable and punctual regarding scheduling and attendance.
  • *Adhere to all applicable State and Federal regulations.
  • Display a courteous, caring, and professional attitude at all times to the clientele, volunteers, and visitors of Heartland Family Service.
  • Abide by all specific program and Agency procedures, policies and requirements.
  • *Create, maintain and share as appropriate a dynamic self-care plan.
  • * Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • * Essential functions of this job are to be performed on company physical work site or designated workspace.
