Vermont’s Free & Referral Clinics (VFRC) is a state-wide association serving a network of seven healthcare clinics and programs across the state that provide free services. The association serves as a voice at the state level for free clinics as well as patients across Vermont affected by barriers such as high healthcare costs, limited access to primary care, lack of insurance, and lack of proficiency with English.

VFRC’s Bridges to Health Program is recruiting three bilingual Community Health Workers to improve access to information and health care services for individuals and families who struggle to navigate health care systems, and are uninsured, geographically isolated, or otherwise in need of being connected to community resources.

Position 1: Community Health Worker – Benefits Navigator

The Benefits Navigator will implement a community-based health benefits outreach and enrollment initiative to reach individuals and families who face challenges navigating health care systems. This CHW role guides income qualifying individuals and families through eligibility, enrollment and access to a range of federal, state, and community programs that help address individual health care costs. They will work collaboratively with the Community Health Worker team to gather needed documentation and submit complete applications. They will also curate and disseminate existing resources, developing new materials as needed, to improve knowledge about and comprehensive utilization of assistance programs.

This is a hybrid/remote flexible position (Vermont Residents Only).

Positions 2 & 3: Community Health Worker – Regional

The regional Community Health Worker will implement community-based outreach activities that ensure community members in their region receive information about health resources available to them. This role will improve quality of life at an individual, household and community level by serving as a liaison, educator, navigator, care coordinator, and interpreter between individuals and community-based organizations to promote health and improve service delivery. They will coordinate clinical outreach efforts and collaborate with local health and social service organizations to assist them in enhancing services and addressing long standing challenges that affect access to care.

Candidates for the regional Community Health Worker positions must reside in one of the following Vermont counties: Caledonia, Essex, Lamoille, Orange, Orleans, and Washington.

Requirements and Preferred Qualifications

    • Bachelor’s degree in social sciences or humanities and three to four years of related experience; or equivalent combination of education and experience.

    • Applicant must be bilingual in English and Spanish with strong speaking, listening, reading and writing skills in both languages.

    • Ability to effectively and independently plan, organize, and coordinate access to health services.

    • Attention to detail, strong interpersonal and communication skills required to work and empathize with diverse audiences.

    • Leadership skills and ability to use judgment in prioritizing and responding to competing work demands.

    • Strong technology, computer, multimedia, and social media skills are essential.

    • Experience with rural families and/or populations that experience challenges accessing needed health related services.

    • Professional or personal familiarity with navigating health care access in Vermont.

    • Certified Application Counselor and Medical Interpretation training a plus.

    • Valid driver’s license, favorable driving record, access to dependable transportation and a willingness travel within Vermont.

    • Willingness to, on occasion, work a flexible schedule that could include evenings and weekends.

Working conditions

Work for this position is conducted at a home or shared office, in clients’ homes or workplaces, and at community locations. At times, home visits are in an agricultural setting with exposure to farm sights, sounds, smells, and equipment. The ability to physically navigate these various environments is required. The work involves remaining in a stationary position for extended periods of time while working on a computer. It may require light to moderate physical activity including occasional bending, stooping or reaching for items and
lifting of up to 25 pounds.

Compensation

This is a full-time, exempt position with competitive pay from $24.00 to $30.00 per hour commensurate with experience and qualifications. Benefits include generous paid time off, a retirement plan, a Health Reimbursement Arrangement towards insurance premiums and a Health Savings Account.
